Code to improve the io recovery time. IO recovery time can extend into
the 90 second range.  This ifix will cut down the recovery time of the
following error injections:

- Concurrent down load
- Online/Offline
- Controller Reset
- sysReboot

It is important to note that there are times that the DS3, 4, and 5K
controllers will return a status of not ready when the disk driver
attempts to reinstate io. This can cause delays and nullify the
improvements in this ifix. In tests IO recovery time averaged between
30 and 40 seconds.
